NEW VACANCY – Patient Care Manager – JHB
Responsibilities:
Medical management of all in and out-patients.
Monitor, coordinate, and implement patient treatment plans (clinical assessment and clerking). Ensuring that treatment plans are executed by Hospital Nursing Staff (in-patients).
Patient education and support on the diagnoses/ prognoses.
Providing guidance and assistance on treatment plans, regular check ins with patients’
regarding health status and wellbeing.
Post hospitalisation check-ins.
Management and identification of ‘red flags’ in the medical results (pathology, radiology, and any other diagnostic test results) of patients and notification to the doctor.
Supporting Doctor during all hospital ward rounds by capturing and scanning all patient notes (ward charts, clerking sheets, doctor’s notes).
Responsible for daily liaison and provision of feedback on patient condition and progress to family member/s of the patient.
Mitigation any risk(s) in terms of patient care satisfaction.
Medical consumable stock management for nursing station and examination room.
Assisting with administrative duties – medical insurance claim forms, chronic illness applications, letters of motivation, preparation of prescriptions, death certificate preparation.
Management of infection control in the practice. Implementation of infection control protocols and safety standards in the practice.
Requirements:
Registered Nurse.
Minimum of three years in a nursing role.
Previous experience in a nursing role in a doctors’ practice advantageous.
ICU experience.
Experience working in a specialist practice will be an advantage.
MS Office – Word, Excel, Outlook.
